ALL CONTRACTORS – FISCAL REQUIREMENTS
Each vendor must have a “Substitute Form W-9” on file electronically with the Florida Department of Financial Services (DFS). If you have not already submitted a “Substitute Form W-9” online, please visit the DFS website to create a profile and upload the information for your “Substitute Form W-9.” W-9 Requirements

You must also register as a vendor with the State of Florida. Register at My Florida Marketplace

Accounting Forms

The following invoice forms are required by the Office of the State Courts Administrator (OSCA) to receive payment for services provided to the trial courts in the Fifth Judicial Circuit, which includes Citrus, Hernando, Lake, Marion and Sumter counties.
