Certified Process Server Application Requirements

Below you will find the process server application per Administrative Order A-2008-21-C regarding policy and procedure for certified process servers. The administrative order requires the following for consideration and placement on the approved list of Certified Civil Process Servers for the Fifth Judicial Circuit.

Fifth Judicial Circuit Certified Process Server Courses 2025

The process server course is a 16-hour training seminar covered over two days. The online class is taught live via Zoom.  You must attend both days of the class online with a webcam and complete the exam to receive a certification of completion from the trainer.

Required documents include: (ALL items (except Training fee) must be received before scheduling a fingerprint appointment)

Process Server applications and other required documents will be accepted through September 26, 2025.

  • Complete Application and Certificate of Good Conduct/Oath of Office
  • Payment of $250.00 payable to the STATE OF FLORIDA (Non-Refundable). You may also pay this application fee online.
  • Level II background check (Appointments can be made by contacting 352-638-1911. NOTE: All documents and application fee MUST be received prior to scheduling a fingerprint appointment.
  • NOTE: Bring fingerprint form and authorization at the time of printing. Fingerprint dates (09/03/2025 & 09/29/2025 from 1PM- 3PM)
  • Training fee of $155.00 (Payable separately/directly to the trainer. Discount code will be provided upon approved background check. DO NOT include in your packet.)
  • All documents can be emailed to process@circuit5.org and proof of payment of the application fee.

Once all documents are received (including Level II background check), training information will be released on an individual basis.

Certified Process Server Renewal Requirements

Below you will find the items required for renewal

·         A Surety Bond of $5,000.00, showing the beginning and expiration date

·         A completed and notarized Certificate of Good Conduct and Oath

·         $150.00 renewal fee. Check or Money Order should be made payable to the STATE OF FLORIDA or click here to make your payment online.

The completed items must be submitted to: Certified Process Server Program Coordinator c/o Court Administration, Lake County Courthouse, PO Box 7800, Tavares, FL 32778 or emailed to process@circuit5.org  prior to expiration date.
